Summary

This short prospective investigation contrasted the findings of 45 healthy, asymptomatic, age-matched, vaccinated groups and performed thorough immune profiling on 16 young people and adolescents hospitalized for myocarditis after severe acute respiratory syndrome coronavirus 2  mRNA immunization. Most individuals in the post-vaccination myocarditis group were men who experienced myocarditis after receiving the second dose, with symptoms appearing one week after vaccination.

No differences in the vaccine-induced host defense between people who experienced myocarditis and those who did not were discovered. Nevertheless, the post-vaccination myocarditis cohort showed higher free, unlimited spike antigen values.

With a slight rise in cytokine production, the comprehensive antibody profiles and T-cell activities in those who experienced post-vaccinal myocarditis were virtually identical to those in the vaccinated comparison group. In addition, considerably higher levels of comprehensive spike protein were present in the plasma of individuals with post-vaccination myocarditis, although this was not connected to antibodies. In contrast, no free spike was found in the bloodstream of asymptomatic, immunized, healthy participants.

Immuno-profiles of immunized adolescents and young adults showed no difference in the mRNA vaccine-induced immune reactions between those who developed myocarditis and those who did not. Conversely, post-mRNA vaccine myocarditis in teens and young adults with free spike antigens in their blood provided new information about its possible etiology. A distinctive marker that could factor into the unexpected reaction of myocarditis post-mRNA vaccination was not present in any healthy, unvaccinated subjects.

Immuno-profiling performed on the participants who developed post-vaccination myocarditis revealed high spike antigen levels without signs of autoantibody generation, concurrent viral diseases, or high antibody reactions to the SARS-CoV-2 mRNA vaccine. However, additional research is necessary to comprehend the immunologically mediated pathways underlying post-vaccination myocarditis.

We must optimize therapeutic and preventative methods across all age groups to protect everyone’s short- and long-term health since COVID-19 remains a chronic condition.
